[EP-tech] Feedback story

Dear all,

i was searching for examples of "first page" for implementing "Coverpage" plugin, and i was very well impressed by Harvard one, that presents in its "coverpage" a link to a form through which
the reader can describe why that paper has been useful for his research.

I think that this simple form could be interesting to implement, both for doing a sort  "reader statistic" and for authors feedback.
I was wondering if anybody has already created a plugin to do this.
